Future of Care : dementia

SJD Social Innova, the social innovation programme of Fundació Sant Joan de Déu (FSJD), is interested in forming a core group of committed partners to jointly prepare a strong proposal for this two‑stage call. 

SJD Social Innova works directly with people with dementia and their informal caregivers through a network of health, long‑term care and social service providers, addressing the intertwined challenges of ageing, dependency, care burden and socio‑economic vulnerability. Our work spans care provision, community‑based support and labour‑care balance, with strong attention to gender and inequality dimensions. 

Our proposed contribution and role in the core group: 

• Co‑designing person‑centred interventions together with people with dementia, informal caregivers and care professionals, focusing on autonomy, inclusion and quality of life; 

• Piloting integrated care and support models that strengthen coordination between health care, long‑term care, social services and community actors; 

• Testing support pathways for informal caregivers, addressing emotional burden, economic resilience, work–care balance and access to services; 

• Contributing real‑world evidence from care settings to inform policy‑relevant, scalable and transferable solutions. 

SJD Social Innova brings direct access to care providers and informal carers, enabling meaningful user involvement and real‑life experimentation throughout the project lifecycle. We have experience working in European collaborative projects and supporting the translation of practice‑based knowledge into policy‑relevant outputs. 

Our intention is to co‑shape the proposal from the outset, contributing implementation capacity, piloting sites and social innovation methodologies, rather than acting as a late‑stage add‑on partner
