Longterm care policy: challenges for carers and integrated health systems

SJD Social Innova, the social innovation programme of Fundació Sant Joan de Déu (FSJD), is interested in participating in this call as a partner, contributing real‑world piloting and social innovation expertise focused on long‑term care systems and care relationships under demographic pressure. 

In this topic we can contibute to: 

• Understand the challenges faced by carers in the context of increased care responsibilities, gender inequalities and burnout. Contribute practice‑based evidence on how long‑term care policies affect accessibility, quality of care, carers’ wellbeing and system sustainability. • Evaluate integrated care pathways that link health, social services and community. In the context of demographic changes and the role for active and healthy ageing policies. (through the Parc Sanitari Sant Joan de Déu https://www.pssjd.org/)) 

SJD Social Innova brings access to a network of long‑term care providers and care services, as well as close collaboration with informal carers, enabling real‑world validation and transfer of long‑term care innovations across contexts. 

We aim to strengthen consortia seeking credible implementation capacity, user‑centred innovation and applied evidence to support more resilient, equitable and sustainable long‑term care policies in Europe, rather than project coordination.
