Fully Digital and Efficient Primary Health Center of the 21st Century

EAP Sardenya is the primary health service of the Baix Guinardo neighbourhood in Barcelona. We are celebrating 25 years of innovation to improve quality assistance, diagnosis, treatments, homecare attention and customer service.

The center is planning to move to a brand new and wider building and we want to design it incorporating the best and innovative solutions available to make it an european model of good practice on the following issues:

• Quicker and cheaper Diagnosis systems using digital tools both at the Center level and remote (at home)

• Improving patient and users customer service and attention at the Center

• Transforming daily management systems

• Incorporating efficient instruments on climate and energy use

• Homecare and health solutions to prevent hospitalisation and institutionalisation

• Digital and IA solutions to make our services fully accessible for people with disablities or special needs

• Tools to better connect healt and care services provided at home
