About
KanopyMed is seeking to join a Horizon Europe consortium as a clinical/technology partner for the 2026 Health Cluster calls, in particular HORIZON-HLTH-2026-01-DISEASE-15 and HORIZON-HLTH-2026-01-CARE-03.
We offer PREDIC, a validated and CE-marked (class IIb) Software as a Medical Device (SAMD) designed to personalize post-hospital care pathways for patients with chronic heart failure. PREDIC supports risk stratification during hospital admission and recommends individualized care pathways after hospital discharge, with the objective of improving outcomes while optimizing healthcare resource use.
We are looking to engage with coordinators and partners (academic hospitals, clinical networks, HTA experts, health economists, social sciences teams, patient organizations) interested in launching a multicenter prospective study to assess the real-world impact of PREDIC. Outcomes of interest may include one or more of the following:

Clinical outcomes (e.g. readmissions, morbidity, patient-reported outcomes)

Organizational impact and care pathway redesign

Efficiency and resource utilization

Access to care and reduction of inequalities

Acceptability, usability and adoption by patients and healthcare professionals

Identification and reduction of low-value care
KanopyMed will contribute the SAMD, clinical and regulatory expertise, and experience in digital health evaluation. We are open to different study designs and implementation contexts across European healthcare systems, with the ambition to support scalable, evidence-based innovation in cardiovascular care. If necessary, we can also bring a network of 10 + teaching and non-teaching hospitals in France.
We welcome discussions with potential consortium partners and coordinators interested in co-building a competitive Horizon Europe proposal.
