About

We are a non-profit healthcare organization operating a multi-service long-term care infrastructure providing residential, semi-residential and home care services for elderly, disabled and frail patients.

The organisation includes a nursing home, Alzheimer’s centre, intermediate care and home assistance programmes, ensuring continuity of care between hospital and community settings. We support people with chronic conditions, neurocognitive disorders, disability and multimorbidity, involving healthcare professionals, caregivers and families in daily care pathways.

Our facilities have recently undergone renovation and energy efficiency improvements, including the adoption of renewable energy solutions, contributing to environmentally sustainable and resilient care services. This allows us to explore and support healthcare models that combine quality of care with environmental sustainability.

Our structure represents a real-world environment for testing and validating innovative health and care solutions addressing ageing, chronicity and integrated care. We can host pilots involving healthcare staff and patients, collect user feedback, evaluate usability and support co-creation activities with end-users. We provide access to real care workflows, multidisciplinary professionals and long-term observation of patients in operational conditions.

We aim to join a consortium as an end-user and pilot site, supporting real-world validation, recruitment of patients and caregivers, acceptability assessment and implementation feasibility analysis. The foundation can contribute to project activities related to:

  • pilot implementation in long-term care and community care environments

  • validation of digital, organisational or assistive health solutions

  • recruitment and involvement of elderly, disabled and chronically ill patients and caregivers

  • co-creation workshops and user experience evaluation

  • assessment of usability, acceptability and adherence

  • support to ethical procedures and informed consent

  • collection of qualitative and observational real-world data

  • evaluation of care pathways for patients with multimorbidity

  • testing of sustainable healthcare practices in care facilities

  • dissemination and stakeholder engagement within local care networks
