Looking for a COORDINATOR: AI-driven health & social care platform for elderly cancer patients (SENECON)

About
We formed a Horizon Europe consortium around SENECON, an AI-driven platform designed to improve outcomes for elderly patients undergoing cancer treatment by integrating clinical, functional, and social care data into a unified system.
The solution aims to move beyond isolated clinical tools by enabling:

prediction of treatment tolerance and complications

monitoring of functional decline and quality of life

integration of social determinants and care dependency
-
coordination of care pathways across oncology, rehabilitation, and social services
The platform is based on a Health & Social Twin approach, combining real-world data with predictive modelling to support more personalised and proactive care for ageing populations.
We are currently looking for a coordinator to join the consortium, particularly:

hospitals and oncology centres for clinical validation
-
digital health / AI companies for platform development
-
health IT providers for system integration
-
organisations working in ageing, social care, and patient engagement
-
research partners in data science, health economics, and care models
The project targets Horizon Europe Cluster 1 calls and aims to reach TRL 6–7 through real-world pilot validation.
