Romanian Living Lab for Remote Monitoring, Hospital-at-Home and Integrated Care | UNITE Call 2

Prevencia is building a Romanian real-world living lab for mature digital health solutions that support personalised remote care, hospital-at-home and integrated patient pathways.

Together with Puls, Colegiul Pacienților and our healthcare partners, the living lab can connect:

• primary care and multidisciplinary specialist services;

• medical imaging and laboratory diagnostics;

• ambulance and medical transport;

• homecare and remote follow-up;

• patient recruitment, engagement and structured feedback;

• clinical staff, workflow and usability evaluation.

We are seeking consortium leaders and technology partners with TRL 6–8 solutions in areas such as:

• remote patient monitoring, wearables and ePROs;

• Software as a Medical Device, clinical decision support and AI-enabled triage;

• hospital-at-home, virtual wards and hybrid-care platforms;

• digital therapeutics and chronic-disease management;

• interoperability and healthcare workflow automation;

• integration of clinical, imaging, laboratory and patient-generated data.

Prevencia and its partners can contribute to pilot design, clinical pathway integration, recruitment of relevant users, clinician and patient feedback, usability and adoption assessment, real-world evidence generation and dissemination. All clinical activities would be conducted subject to the appropriate ethical, legal and data-protection requirements.

We are particularly interested in proposals addressing UNITE's challenge on more personalised and human-centred remote care, with potential links to equal health access, workforce efficiency and deep-tech innovation.

Possible roles include clinical pilot site, end-user organisation, Romanian regional implementation partner and real-world validation partner.
