About
Having tools that allow us to move towards a Person-Centered Care (PCC) model
Fundació Hospital Torroella de Montgrí presentation
Fundació Hospital Torroella de Montgrí is a non-profit social organization committed to caring for older people in situations of dependency and to supporting their families.
The Foundation is located in the municipality of Torroella de Montgrí–l’Estartit and manages two residential facilities: Vila Vella Residence and Santa Caterina Residence and Day Care Center, with the fundamental objectives of providing the best possible service to residents and the best support to their families.
Both residences are classified as assisted living facilities, in accordance with the protocol signed with Departament de Drets Socials i Inclusió, and they are centers accredited by the Government of Catalonia (Generalitat de Catalunya).
Current Context
The Foundation is currently about to complete the renovation works of the Vila Vella Residence, which will allow us to create stable cohabitation units (5 units) with a maximum of 15 residents each. The main goal is to improve the service we provide and the care offered to residents and their families, moving towards a model that respects the life projects of the people who choose to live in our residential centers, as well as the families we support.
At the same time, these stable cohabitation units will improve the working conditions of our professionals by creating a safer, healthier environment adapted to residents’ needs, enabling them to provide appropriate care.
We believe that at some point, in some space, the Open Innovation Challenge of the Health Revolution Congress 2026 should consider the new residential care models for older people in situations of dependency.
At present, we have completed the second phase in the definition of the Strategic Plan of the Torroella de Montgrí Hospital Foundation 2026–2031, which has allowed us to reflect on the current situation of our organization and to establish its strategic direction for the next five years.
In this regard, all strategic actions of the organization will be based on five key vectors:
1. SERVICE
2. PEOPLE
3. FACILITIES
4. TERRITORY
5. GOVERNANCE
Vector No. 1, SERVICE, is closely linked to our vision as an organization:
We want our centers to provide a respectful and sensitive service aligned with the life projects of residents and day care users.
Identification of the Need
The first vector of the Strategic Plan is linked to the service we aim to provide.
The first objective we want to work on is to consolidate our own care model based on Person-Centered Care (PCC), ensuring that the declared principles are translated into concrete and measurable daily practices.
This model will integrate:
- Autonomy and respect for each person’s life project
- Incorporation of life history into care planning
- Support during deterioration processes and end-of-life stages
- Humanization of care and environments
Definition of the Challenge
We are interested in improving physical and sensory stimulation activities offered to older adults living in our centers who have a higher degree of dependency.
Often, it is easier and we have more resources to offer stimulation activities to older people who have a lower level of dependency. However, it is more difficult for us to provide pleasant, stimulating, and meaningful activities for those older adults who, due to illness, present severe physical and cognitive deterioration.
Therefore, we are interested in learning about the resources (technological, human, material, structural, and social) that companies can offer in order to provide physical, cognitive, and sensory stimulation activities for older adults whose illnesses involve a higher level of dependency.
