About

Doole Health designs and delivers digital health solutions that support better, more coordinated care across healthcare systems. Our work is based on a modular, white-label digital platform that can be configured to support a wide range of care programmes, service models and clinical contexts. The platform is designed to fit everyday clinical workflows, support continuity of care beyond hospital settings and transform patient interactions into structured, actionable data for healthcare professionals.

Our approach is strongly implementation-driven. We work through close co-design with clinical teams, interoperability with hospital information systems and solutions conceived to scale beyond the lifetime of a project. The platform integrates digital care pathways, patient engagement tools, secure communication and AI-enabled support for clinical decision-making and operational efficiency.

Horizon Europe track record

ROSIA PCP Rehabilify
Doole Health participated in the ROSIA PCP as technology partner. The resulting solution, Rehabilify, a digital rehabilitation platform built on Doole’s technology, successfully completed its Phase 3 pilot. It was validated across three European healthcare systems—SALUD Aragón (Spain), NRH (Ireland) and CHUC (Portugal)—and evolved from prototype to a market-ready product through iterative co-design with clinical teams. Rehabilify supports deployment across multiple pathologies and care settings, integrates eight third-party services and follows a dual-market approach (Rehabilify Pro and Lite). Regulatory certification is currently in its final stages.

CareMatrix PCP INCA Healthcare
Within the CareMatrix PCP, Doole adapted its platform to develop INCA Healthcare, a next-generation digital solution for integrated care in multimorbidity. INCA combines a patient and caregiver mobile app with a professional web interface, enabling shared care plans, secure communication, alerts and decision support. The platform integrates risk stratification, validated assessment tools, AI-driven decision support, gamification and an AI coach to strengthen engagement and adherence. INCA is currently closing Phase 3, with a SaaS exploitation model including risk-sharing schemes and regulatory steps underway towards CE marking as a Class I medical device.

AI-driven innovation projects

Beyond Horizon Europe, Doole is involved in international R&I projects applying AI to concrete clinical and operational challenges.

Neprem-Care (CDTI Multipaís)
Development of AI-based predictive models to anticipate unplanned hospital readmissions and surgical cancellations, using electronic health records and operational data. The models are validated in real clinical environments and integrated into hospital systems to enable predictive alerts and proactive interventions.

OPTIMUS – Advanced AI Platform for Perioperative Management
OPTIMUS focuses on an AI-powered platform integrating personalised prehabilitation and rehabilitation, patient education, mental health support, tailored physical interventions and continuous symptom monitoring. The project aims to reduce fragmentation in perioperative care, improve outcomes and optimise hospital resource use through predictive analytics and remote patient management.

RETECH – CUIDA+ (LINA)
Within the RETECH programme, Doole is developing CUIDA+ (LINA), an AI-powered multichannel virtual assistant designed to reduce the digital divide in healthcare. Accessible via voice and messaging, LINA converts natural language into structured clinical data using generative AI (LLM + RAG), supporting hospital-at-home and remote monitoring programmes without requiring complex digital skills or continuous internet access.
