AI-enabled personalised care pathways for preventive and precision medicine

AI-enabled digital health platform designed to support preventive, precision and personalised medicine by integrating multimodal health data, including clinical records, laboratory biomarkers and natural language data from medical interactions.

The solution combines large language models (LLMs), advanced analytics and clinical evidence to deliver context-aware, explainable and trustworthy decision support for healthcare professionals, targeting high clinical acceptance and seamless integration into care pathways. A specialised automatic speech recognition (ASR) and NLP module enables real-time transcription and analysis of doctor–patient interactions, improving clinical efficiency and data quality.

Our tool aims to demonstrate measurable health impact, including improvements in biological age-related indicators and risk profiles, through a real-world pilot case–control study. The platform is designed to align with EU priorities on prevention, healthy ageing, digital transformation of healthcare, trustworthy AI, and the European Health Data Space, supporting scalability and adoption across European health systems.

The project is seeking clinical, academic and industry partners to collaboratively address challenges in AI-driven healthcare delivery, disease prevention and sustainable health systems, in line with Horizon Europe Cluster Health calls.
