About

The Elisabetta Germani Foundation is an italian private non-profit organization operating in the social and healthcare sector, with a 126 years experience in delivering integrated services for vulnerable populations. Its activities span the entire care continuum, including: residential care, day care services, outpatient support, rehabilitation and home-based care, ensuring continuity and coordination across different levels of need and care settings.

The Foundation provides services for older adults, people with disabilities and individuals living with cognitive decline, adopting person-centered, inclusive, and holistic approaches. Its care models integrate health, social and psychosocial dimensions, with the aim of supporting autonomy, quality of life, and continuity of care over time. Particular attention is given to long-term care pathways, dementia and cognitive impairment and disability support, both in residential environments and in community- and home-based contexts.

As an end user, the Foundation operates in real-life service delivery environments and represents the perspective of care providers, professionals, service users, and informal caregivers. It contributes practical insights into care processes, organizational dynamics and user needs across different care settings, from institutional contexts to domiciliary services. The Foundation is therefore well positioned to support user-centered design, co-creation activities, pilot implementatio and validation of innovative solutions addressing real-world health and care challenges.

The organization values multidisciplinary collaboration, ethical and human-centered innovation, and solutions that are feasible, acceptable, and sustainable within everyday care practice. Within Horizon Europe Cluster 1, the Elisabetta Germani Foundation seeks to participate as a pilot site in projects focused on ageing, disability, integrated health and social care, long-term and community-based care and the transformation of care systems, contributing to the translation of innovation into meaningful and scalable impact for people and services.
