At WoodChill we provide nature-based wellbeing programmes and are developing services that can sit within social prescribing pathways, supporting people before, alongside and beyond traditional healthcare.

One of the problems we see is that community and non-clinical services can make a real difference to people’s wellbeing, but referrals, attendance and outcomes are often fragmented or difficult to evidence. Healthcare professionals may refer someone into a community service, but there is not always a simple way to see whether they attended, how they progressed or what difference the intervention made.

We want to develop a digital social prescribing and wellbeing platform that connects healthcare, community organisations and wellbeing providers.

The platform would allow referrals to be made and managed digitally, record attendance and engagement, collect simple before-and-after wellbeing measures and follow participants over time to understand the wider impact of non-clinical interventions.

The aim is not simply to create a booking app. It is to create a system that helps demonstrate what works, for whom and what happens after someone is referred, while giving healthcare and community partners useful evidence of impact.

WoodChill would provide the real-world service environment and pilot population in which the solution could be developed and tested.

We are looking to connect with a digital or technology partner to develop the platform, a research or university partner to help design and validate the outcome measurement, and healthcare and social prescribing partners to help shape the referral pathway and test how the system could work in practice.

The longer-term ambition is for the platform to be usable beyond WoodChill by other community wellbeing providers working within social prescribing pathways, creating a clearer link between healthcare referrals, community intervention and measurable outcomes.
