About

eSalut+ proposes to advance personalized medicine by expanding Bellvitge University Hospital's proven eSalutMetroSud web application. This integrated platform provides patients with a centralized and validated health information site, PROMs/PREMs direct reporting, and tailored healthy lifestyle/exercise recommendations. Clinicians benefit from a robust tool to track patient evolution and support individualized care.

Our objectives are to enhance patient engagement, optimize clinician decision support through advanced analytics, and expand data integration for holistic patient profiles. We aim to refine existing functionalities and develop new modules to deliver highly tailored interventions, improving patient outcomes, self-management, and healthcare efficiency. This project directly addresses PRECISEU's personalized medicine challenge by leveraging a scalable digital health solution to foster a more proactive and patient-centric healthcare model. Expected outcomes include improved patient adherence, enhanced clinical insights, and optimized resource utilization. Approximate duration: 36 months.

The eSalut+ project stands out due to several innovative aspects that promise a significant impact on personalized medicine, directly benefiting both patients and healthcare professionals, and contributing to the broader healthcare sector. 

  • Effective Patient Empowerment: It leverages rich audiovisual content, personalized recommendations, adapted exercises, and interactive tools, all meticulously tailored to individual patient needs. This approach fosters active participation in their health management, promoting self-efficacy and adherence to treatment plans. 

  • Facilitating Personalized and Continuous Follow-up: The platform enables proactive monitoring, encouraging self-care and significantly reducing acute exacerbations. By providing continuous, personalized insights, it shifts the healthcare paradigm from reactive to preventive, allowing for timely interventions. 

  • Real Integration with the Clinical Environment: A cornerstone of eSalut+'s innovation is its seamless, real-time integration with the clinical workflow. Healthcare professionals can visualize patient-generated data, track evolution, monitor interactions, and receive critical alerts instantly. This immediate access to comprehensive patient information supports highly informed and personalized clinical decision-making. 

  • Total Interoperability: eSalut+ boasts full interoperability, ensuring its adaptability to existing hospital systems such as SAP and Electronic Health Records (HCE), as well as adherence to institutional standards. 

  • Structured Evaluation of Experience and Outcomes: The project incorporates a rigorous, structured evaluation framework through the use of PROMs (Patient-Reported Outcome Measures), PREMs (Patient-Reported Experience Measures), biomedic measurements, and validated questionnaires. This systematic collection of patient-centric data provides invaluable insights into the effectiveness of care and patient satisfaction, driving continuous improvement. 

  • Modular and Future-Ready Architecture: eSalut+'s modular design is a key innovation, allowing for the flexible incorporation of new functionalities. This adaptability is crucial for evolving personalized follow-up strategies, prioritizing interventions based on real-time data, and analyzing complex patterns of patient behavior and clinical evolution. 

Within the scope of this project, we plan to additionally explore the integration of Artificial Intelligence (AI), with the support of specialized technological partners. The objective is to evolve towards a smarter care model, even more adapted to the patient's profile and real needs. This will significantly enhance clinical impact, optimize resource utilization, and support decision-making for both professionals and patients, particularly in data analysis and facilitating personalized clinical decisions. 

Technology Readiness Level (TRL): 

  • Current TRL: 7. System prototype demonstration in an operational environment, in real use with patients in a clinical setting (Bellvitge University Hospital). 

  • Target TRL: 9. Actual system proven in operational environment, validated deployment in diverse European clinical settings.
