LifePath moves beyond static, diagnosis-based disability service models by establishing a scalable, practice-based ecosystem of adaptive community pathways. Unlike traditional linear or one-directional approaches, LifePath introduces a reversible, dynamic pathway logic tailored to the individual's evolving lifecycle needs, safety, and autonomy.
Individuals transition flexibly across interconnected environments based on a structured, multi-dimensional assessment of their functional capabilities, vocational readiness, and personal aspirations over time.
Integrated Service Ecosystem & Infrastructure
Operating as a fully operational, de-risked Living Lab validation environment in Cyprus (Saint Lazarus Centre), LifePath integrates:
Vocational Rehabilitation & Supported Employment:* Providing structured vocational readiness metrics, digital upskilling, and a proven Supported Employment Model that bridges the gap to the open, inclusive labor market.
Independent Living Models (CityHome):* Transition frameworks and evidence-based pathways connecting supported living to autonomous community residency.
Day Centre Services:* Community-based programming focusing on social/cultural participation, lifelong learning, and stability during periods of transition.
Evaluator-Aligned Impact & EU Policy Alignment
The LifePath framework directly operationalizes key EU disability, employment, and long-term care strategies, making it an ideal pilot site for Horizon Europe Cluster 2 proposals (specifically targeting Inclusive Labor Markets and Social Transformations). It delivers:
