MetaCare: patient and caregiver centered digital care models in onco-hematology

About

We are a multidisciplinary team composed of hematologists and expert hematology nurse case manager working in a comprehensive cancer care setting (IRCCS).

Our experience is rooted in the design and implementation of integrated clinical and care pathways for onco-hematological patients, with specific attention to continuity of care, complexity management, and the psychosocial burden experienced by patients and their caregivers.
Over the years, we have been involved in clinical research, observational studies, organizational projects, and educational initiatives aimed at improving quality of care and quality of life.

METACARE is an ongoing project developed within this context, focused on supporting cancer patients and caregivers through innovative, patient-centred solutions. The project integrates clinical expertise, nursing case management, psychological support, and digital health approaches, including immersive technologies and AI-based tools, with the aim of reducing distress, preventing caregiver burnout, and enhancing patient and caregiver empowerment along the care pathway.

Within a potential Horizon Europe proposal, we can contribute with:
• strong clinical and nursing expertise in onco-hematology and complex care pathways
• experience in patient- and caregiver-centred care models
• real-world clinical settings for pilot testing and implementation
• co-design methodologies involving patients, caregivers, and healthcare professionals
• outcome evaluation using clinical, organizational, and patient-reported measures.

We are looking for cooperation with universities and research centres, digital health and immersive technology SMEs, patient organisations, and healthcare providers interested in co-developing, testing, and scaling innovative solutions in cancer care.
Our team is available to act as clinical partner, pilot site, or work package leader, particularly in areas related to patient and caregiver engagement, real-world implementation, and impact assessment.
