Partnering to Prevent Polypharmacy Adverse Drug Reactions via Personalized Care

European Project Coordinator at Biokeralty Research Institute AIE

About

Keralty is an international integrated health group with over 6.9 million patients and 24,450 professionals across Colombia, Peru, Mexico, and the United States. Our organisation combines healthcare delivery, diagnostics, digital health, and research, with extensive experience participating in H2020 and Horizon Europe collaborative frameworks.

Keralty is exceptionally well positioned to contribute to HORIZON-HLTH-2027-01-CARE-02, supporting personalized approaches to prevent and reduce Adverse Drug Reactions (ADRs) resulting from polypharmacy (concomitant administration of three or more medications), particularly in older patients and complex multimorbid populations. Our key capabilities include:

  • Polypharmacy Cohorts & Clinical Validation: Direct access to diverse, longitudinal patient populations and Real-World Data (RWD) to conduct observational and interventional clinical studies on polypharmacy, prescription cascades, de-escalation strategies, and sex-specific ADR differences.

  • Pharmacogenomics & Multi-Omics Infrastructure: A network of over 70 laboratories equipped with advanced genomic and molecular diagnostic technologies to profile drug-drug, drug-gene, and pharmaco-exposomic interactions, as well as validate predictive biomarkers for personalized drug therapy optimization.

  • AI, EHR & Clinical Decision Support Systems (CDSS): Expertise in deploying AI-driven clinical decision-support algorithms integrated into Electronic Health Record (EHR) systems to flag adverse drug interactions, prevent inappropriate prescribing cascades, and support personalized medication management at the point of care.

  • Implementation Science & Real-World Care Integration: Proven capability to evaluate intervention clinical utility, cost-effectiveness, and adoption within primary and specialized care pathways across health systems.

In addition, BioKeralty Research Institute—the R&D&I unit of Keralty—provides specialized support in pharmacogenomics methodology, health economics/cost-effectiveness modeling, EMA regulatory alignment, FAIR data stewardship, and policy/educational dissemination for deprescribing and adjusted-prescribing guidelines.

Together, Keralty and BioKeralty provide an end-to-end clinical execution and data integration platform—combining patient access, pharmacogenomics infrastructure, EHR-embedded digital tools, and implementation science—making us an ideal partner for consortia targeting the HORIZON-HLTH-2027-01-CARE-02 call.
