Digital health platform partner (SME) for Horizon Europe Health & Missions (TRL-ready, multi-disease, RWE)

myPatientSpace is a European digital health SME offering a modular, interoperable, TRL- ready platform to support research, innovation, and deployment across Horizon Europe Cluster 1 Health and EU Missions, including the Mission on Cancer.

We bring direct Horizon 2020 experience, including acting as a Work Package leader, with responsibilities spanning technical delivery, pilot implementation, stakeholder coordination, and contributions to evaluation, dissemination, and exploitation. We have strong experience working with leading academic centres, academic hospitals, and healthcare systems across Europe, supporting research-led innovation and real-world implementation. Our work has contributed to multiple award-winning digital health initiatives, supported by clinical evidence and real-world use.

The myPatientSpace platform is designed to support TRL progression, from early-stage pilots and validation through to large-scale demonstration and scale-up within EU-funded actions. It enables remote patient monitoring, patient-reported outcomes (PROMs), digital care pathways, and longitudinal data capture, supporting both research and routine care contexts.

Within the Mission on Cancer, we bring experience in geriatric oncology, with a strong focus on quality of life, survivorship, multimorbidity, and patient support. More broadly, we have extensive experience working with life sciences, pharma, and medtech partners, enabling real-world evidence (RWE) generation, patient engagement, and post-market and implementation research.

We are device-agnostic and integrate with a wide range of leading medical devices and wearables, enabling flexible study designs and applicability across multiple disease areas, including:

• Cancer/Oncology (including geriatric oncology and survivorship)

• Cardiovascular disease

• Renal / chronic kidney disease

• Respiratory disease

• Diabetes and cardiometabolic conditions

• Multimorbidity and ageing-related care

• Any disease area/population (e.g. young patients with NCDs)

The platform aligns with FHIR interoperability standards, GDPR, and MDR-compliant medical software development, and is well suited to large-scale pilots, pragmatic clinical studies, and real-world validation across diverse healthcare and community settings.

Within consortia, myPatientSpace can act as a technology provider, Work Package lead, pilot and validation partner, or exploitation and scale-up partner, contributing:
