AI4HealthAccess: Intelligent Climate‑Informed Personalized Care Pathways.

About

AI4HealthAccess deploys artificial intelligence and climate‑informed health forecasting to strengthen integrated and personalised care pathways for vulnerable citizens. The project will combine AI, Bayesian and spatio‑temporal models with electronic health records, environmental data and digital health tools to predict climate‑sensitive health risks and care needs at individual and population level.

Working with hospitals, primary care, public health agencies and patient organisations, the initiative will co-design decision‑support dashboards and triage tools to improve timely access, continuity of care and resource allocation, particularly for people in vulnerable situations or remote areas. The aim is to deliver scalable, trustworthy AI solutions that enhance resilience of health systems, reduce inequalities in access to high‑quality care and support evidence‑based policies in line with Horizon Europe Destination 4 priorities.
