Smplicare is a digital platform designed to support the early identification and prevention of falls in older adults. It combines continuous wearable monitoring with a mobile application that delivers personalised exercise, behavioural support, and daily guidance, alongside a case management web application where clinicians and wellbeing teams can view risk alerts, monitor individuals, and coordinate targeted intervention.

Central to the platform is the Smplicare Fall Risk Algorithm (SFRa), which integrates longitudinal wearable data (e.g. activity patterns, sleep, and physiological signals) with contextual and behavioural inputs to model each individual’s “normal” baseline and detect meaningful deviations over time. Rather than providing static risk scores, the algorithm identifies dynamic changes in risk trajectory, highlighting acute periods of elevated fall risk days in advance.

This enables earlier, targeted action for higher-risk individuals, such as clinical review, pathway escalation, or tailored intervention, while allowing lower-risk individuals to continue self-managing safely. In doing so, Smplicare addresses a key evidence gap identified in national guidance: the lack of accurate, real-world tools capable of continuously identifying and responding to changes in falls risk in community settings.
