Project Concept
We are preparing a Horizon Europe proposal focused on preventing the progression of type 2 diabetes and reducing its major complications through a multisectoral implementation framework that connects healthcare services, municipalities, community organizations, sports and rehabilitation infrastructures, digital health technologies, and policymakers.
The project aims to develop and evaluate scalable, community-integrated lifestyle interventions supported by AI-based decision tools, biomarker-guided personalization, and implementation science approaches. Particular emphasis will be placed on health equity, sustainability, and large-scale adoption across different European settings.
Partners Sought
